Note: If today’s date is an even number you must submit the top half to the Second Level Authorized Administrator prior to filling out the bottom half using a #2.5 yellow pencil with blue eraser.   If today’s date is an odd number than you may use any #2 pencil with a pink eraser as long as the pencil is not red.
